ABSTRACT

The year 2020 is an extremely unusual year. The world lost more than one million lives due to the attack of COVID-19. Economic production has been greatly reduced, and daily activities are largely restricted. Luckily the work of Chinese Journal of Traumatology (CJTEE) has not been adversely affected. 2020 is a harvest year for the journal, which (1) was included in the high-quality academic journals by China Association for Science and Technology; (2) cover of each issue is newly designed; (3) submission increased by about 60% with more countries and regions covered; (4) usage in the ScienceDirect database exceeded a million; (5) the CiteScore rises to more than 2.0 the first time. This study reviewed the articles published in the year 2020 by CJTEE.

ABSTRACT

The Coronavirus pandemic (COVID-19) triggered a global health emergency, and as a response, institutions and medical services have implemented different measures regarding the management of these patients in order to maintain medical care, and reduce the spread of the virus in patients and medical staff. The following review aims to present the current recommendations made by international, national and local societies regarding medical action from Traumatology and Orthopaedics. A literature review was done on Medline / Pubmed platforms, The Journal Bone and Joint Surgery (JBJS), Journal of the American Academy of Orthopedic Surgeons (JAAOS), Sociedad Chilena de Ortopedia y Traumatología (SCHOT). The literature was in Spanish and English, comparing it with reality on a local level. The current COVID-19 pandemic led to the complete reform of the trauma care units, starting with the development of subspecialty teams that rotate periodically, which are in charge of the emergency area, treating patients with all personal protection elements (PPE) and categorizing those patients who require emergency trauma surgery, from those that can be managed in a delayed manner. Regarding patients who are hospitalized, a COVID-19 PCR (Polymerase Chain Reaction) test is taken as screening, except for emergency surgeries where all intraoperative protection measures are taken. In addition, for greater safety, all elective surgery was suspended, which reduces the flow of patients in the ward and in the inpatient unit. Outpatient care was optimized in order to protect patients in their periodic check-ups.

ABSTRACT

The treatment of severe trauma, especially multiple injuries, requires multidisciplinary collaboration. The current study aims to highlight the challenges of consultation mode for severe trauma in general hospitals and emphasizes the need to create a new temporary-sustainable team. It suggests developing an information consultation mode and enforcing the fine management to improve the quality and safety of the medical treatment. The management mode of a temporary-sustainable team will reduce the cost and improve the treatment efficiency. Overall, a temporary-sustainable team has significant advantages over a traditional multidisciplinary team for severe trauma treatment.

Este estudio pretende reflejar las experiencias, describir y analizar las dificultades en el ensayo de implementar la “gestión por procesos” en el servicio de ortopedia y traumatología de un hospital público de 140 camas como así también reconocer las ventajas y evaluar el impacto sobre la eficacia eficiencia y efectividad. La institución tiene dos pilares básicos en su gestión: brindar prestaciones de alto nivel académico, científico y humano, es decir “compromiso con la Calidad y la Excelencia”, como así también la asunción de la “Responsabilidad y Gestión Social”. Estos dos conceptos se han ido desarrollando a lo largo de la historia de nuestro hospital El objetivo central es afianzar la cultura de la excelencia en la gestión, reordenando y definiendo los procesos y a través de ello lograr: mayor compromiso de los profesionales, alinearse a las necesidades del paciente, descentralizar la gestión, estandarizar las actividades, favorecer la mejora continua, allanar la organización y mejorar la eficacia y eficiencia. Se intenta extraer conclusiones valederas, llevar a cabo una revisión sobre el desarrollo en las distintas fases y etapas de la gestión por procesos, es decir, se vuelve a mirar o “revisita” la institución, sus actores y las experiencias vividas, pasando por avances y retrocesos, aciertos y desaciertos, posibilidades y limitaciones plasmados en los resultados obtenidos. De este modo, se presenta un informe técnico y de reflexión teórica


SUMMARY: This study aims to reflect experiences, describe, and analyze the difficulties in the trial of implementing "process management" in an Orthopedics and Traumatology department of a public hospital with 140 beds as well as recognize the advantages and assess the impact on the efficacy, efficiency and effectiveness. The institution has two basic pillars in its management: to provide benefits of high academic, scientific and human level, that is to say "commitment to Quality and the Excellence", as well as the assumption of “Social Responsibility and Management ". These two concepts have been developing throughout the history of our hospital The main objective is to strengthen the culture of excellence in the management, reordering and defining the process and through this achieve: greater commitment of the professionals, align to the patient’s the needs, decentralize the management, standardize the activities, promote continuous improvements, streamline the organization and improve efficiency and effectiveness. It tries to draw valid conclusions, carry out a review on the development in the different phases and stages of the process management, that is to say, it is re-looks or "revisits " the institution, its actors and their lived experiences, passing through advances and setbacks, successes and failures, possibilities and limitations reflected in the results obtained. In this way, a technical report and theoretical reflection is presented

ABSTRACT

Abstract: This article has two parts. The first discusses the relationship between industry and health interests based on three different but non-mutually exclusive "logics": (a) independent; (b) divergent; and (c) convergent. The second part describes the experience at the Brazilian National Institute of Traumatology and Orthopedics (INTO) with a technology management model. The accumulated expertise in orthopedics at INTO can favor Brazil's domestic medical equipment industry without jeopardizing the country's social health needs. This means directing the production of feasible technologies adapted to the national reality, with a focus on safety and quality, without burdening the public coffers and by overcoming the country's dependency on imported products. The proposal is to promote socioeconomic development through a virtuous circle by attracting reserves and fomenting national competitiveness in domestic and foreign markets while improving social conditions and access to health.


Resumen: Este artículo está dividido en dos partes. En la primera, se discute cómo se relacionan los intereses productivos y la salud, a partir de tres "lógicas" o perspectivas diferentes, que no son mutuamente excluyentes: (a) independiente; (b) divergente; (c) convergente. En la segunda, se describe la experiencia del Instituto Nacional de Traumatología y Ortopedia (INTO) en el montaje de un modelo de gestión de tecnología. El conocimiento internalizado en ortopedia del INTO puede favorecer la industria nacional de equipos médicos, sin abandonar las necesidades sociales brasileñas en salud. Esto es, dirigir la producción de tecnologías viables y adaptadas a la realidad nacional, centrándose en la seguridad y calidad, sin ser onerosos para el erario público y abandonando la dependencia de productos importados. La propuesta es promover un progreso socioeconómico que construya un ciclo virtuoso, con el fin de atraer divisas y fomentar la competitividad nacional en mercados internos y externos, mejorando las condiciones sociales y de acceso a la salud.


Resumo: Este artigo está dividido em duas partes. Na primeira, discute-se como se relacionam os interesses produtivos e a saúde a partir de três "lógicas" ou perspectivas diferentes que não são mutuamente excludentes: (a) independente; (b) divergente e (c) convergente. Na segunda, descreve-se a experiência do Instituto Nacional de Traumatologia e Ortopedia (INTO) na montagem de um modelo de gestão de tecnologia. O conhecimento internalizado em ortopedia do INTO pode favorecer a indústria nacional de equipamentos médicos sem abandonar as necessidades sociais brasileiras de saúde. Isto é, direcionar a produção de tecnologias viáveis e adaptadas à realidade nacional, com foco em segurança e qualidade, sem onerar os cofres públicos e abandonando a dependência de produtos importados. A proposta é a de promover um desenvolvimento socioeconômico que construa um ciclo virtuoso, por atrair divisas e fomentar a competitividade nacional em mercados internos e externos, melhorando as condições sociais e de acesso à saúde.

ABSTRACT

Trauma is a leading cause of death and disability around the world. Injuries are responsible for about six million deaths annually, of which ninety percent occur in developing countries. In Iran, injuries are the most common cause of death among age groups below fifty. Trauma system development is a systematic and comprehensive approach to injury prevention and treatment whose effectiveness has been proved. The present study aims at designing a trauma system management model as the first step toward trauma system establishment in Iran. In this qualitative research, a conceptual framework was developed based on the public health approach and three well-known trauma system models. We used Benchmarks, Indicators and Scoring [BIS] to analyze the current situation of Iran trauma care system. Then the trauma system management was designed using the policy development phase of public health approach The trauma system management model, validated by a panel of experts, describes lead agency, trauma system plan, policy-making councils, and data-based control according to the four main functions of management: leading, planning, organizing and controlling. This model may be implemented in two phases: the exclusive phase, focusing on resource integration and the inclusive phase, which concentrates on system development. The model could facilitate the development of trauma system in Iran through pilot studies as the assurance phase of public health approach. Furthermore, the model can provide a practical framework for trauma system management at the international level

ABSTRACT

Traz a traumatologia buco-maxilo-facial como problema de saúde pública no município de Säo Paulo, a partir da descriçäo da dinâmica da rede de instituiçöes governamentais e vinculadas ao sistema público que prestam esse atendimento, da verificaçäo das instituiçöes quanto ao atendimento e reabilitaçäo e do perfil da morbidade atendida em unidades de internaçöes e unidades de emergência, através da especificaçäo das consultas realizadas, segundo diagnóstico, causa, sexo, idade e instituiçäo onde ocorreu o atendimento. Triou-se as instituiçöes que atendiam a emergência odontológica geral e a traumatologia buco-maxilo-facial através das fontes de dados governamentais do ano de 1996, meses ímpares (SIH-SUS e SIA-SUS) e configurou-se a rede de serviços através de questionários. Encontrou-se 57 por cento dos atendimentos ligados á emergência odontológica comum e 34 por cento à traumatologia buco-maxilo-facial, onde aparece prioritariamente o adulto jovem do sexo masculino, configurado um perfil semelhante ao encontrado na mortalidade por causas externas
